The City of Phoenix supports the health and well-being of its employees by offering valuable wellness resources. These resources include free health coaching, live webinars, a competitive walking program, and a Wellness Incentive for employees enrolled in a City employee medical plan.

Wellness Incentive

Add money to your take home-pay by qualifying for the Wellness Incentive. To qualify, you must complete all three of the following activities within the same calendar year:

  • Have an in-office visit with your in-network primary care physician (PCP): A PCP is a family practice doctor, general practitioner, internist, or OB/GYN in your City of Phoenix employee medical plan network. The visit does not have to be a well visit, but a well visit with an in-network PCP is covered at 100% -- which means it’s free to you.
  • Get your seven pieces of biometric data: Height, weight, waist circumference, blood pressure, blood glucose, total cholesterol, and HDL cholesterol.
  • Complete your online Health Risk Assessment (HRA) using your biometric data.

Complete your HRA online via your current medical plan provider’s portal. You and your spouse (or qualified domestic partner), if any, must each have your own registration on the portal.

The Wellness Incentive applies to employees and/or their covered spouse or qualified domestic partner. When one of you completes the criteria, you receive a $40 per month incentive. When both of you complete the criteria, you receive a $60 per month incentive.

If you don’t fulfill the Wellness Incentive criteria in 2018, you can complete the Wellness Incentive criteria at any time in 2019. It will take about 30 days for your incentive to begin showing up on your paycheck.

Employee Assistance Program (EAP)

The EAP, provided by ComPsych EAP, is a free, confidential benefit to help you and your family members live well, at home and at work. The EAP offers:

  • Confidential support for a variety of issues, including work/life balance, family and relationships, depression and stress management, alcohol/substance abuse, and more.
  • 12 free counseling sessions for you and your eligible dependents or household members, per person, per incident.
  • Referrals to local professionals and services to fit your needs.

The EAP also offers elder care services. A credentialed care manager will come to your loved one’s home to learn more about their situation and needs. After providing an assessment, the care manager will work with family members to develop a customized support plan.

Phoenix Fire Department Employees

If you work in the Phoenix Fire Department in any position, civilian or sworn, you receive EAP services from Public Safety Crisis Solutions (PSCS) at (602) 466-9456 or PSCrisisSolutions.com. The PSCS plan is not administered by the City of Phoenix Benefits Office; it is administered by the Phoenix Fire Department.

Officer Craig Tiger Act (Traumatic Event Counseling) / HB2502

36 counseling session benefit is available to sworn Firefighters and Police Officers who have experienced a traumatic event on duty and need counseling as designated by the Officer Craig Tiger Act/HB2502.
